Please assist us by ensuring that the following information is available at the time of your consultation.
Your referral slip and any X-rays, (if applicable)
A list of medications you are presently taking
If you have dental insurance, bring the applicable card. This will save time and allow us to help you process any claims.
Fees and Payment Options
The fee for your endodontic treatment will be based on the extent of treatment. When you call to make an appointment we will discuss the fees involved. However, this is only an estimate and
you will be charged accordingly when treatment is complete.
We accept assignment of benefits from your insurance company with the following provisions. When you come in for your treatment we will send the claim off to your insurance
company (s) first before we collect anything from you. In the meantime, we require a valid credit card on file for security purposes while we wait for their payment. Once the insurance
company has paid us, we will inform you of how much was covered and your portion will then be processed with the card you left on file (unless other arrangements have been made).
In the event that your insurance company gives us an immediate response at the time of our appointment, we will then collect your difference at that point and delete the credit card
information you provided.
For your convenience, we accept Visa, MasterCard, Cash and Debit.
Financial assistance is available. Please contact our office for details.
*Subject to individual case approval.
Cancellation and Missed Appointments
When you book an appointment with us, we reserve that time specifically for you. As such, we require minimum 2 business days in the event an appointment must be cancelled.
This allows other patients awaiting treatment to be rescheduled into the time slot initially reserved for you.
Short notice cancellations (i.e. less than 2 business days or cancelling appointment on the answering machine) and missed appointments will be subject to fees payable.
